Specific Need

Funds raised through Geary County Match Day will help Delivering Change continue providing free community education, outreach, and resources to local families through events like Luke’s Community Baby Shower and Becoming A Mom Prenatal Education Classes.  Your donation helps families learn about pregnancy health, infant safety, and safe sleep, while providing essential resources such as a safe sleep environment or car seat, to help families put their knowledge into practice at home.  

 

A $50 donation, with the 50% Match Day match, can help provide a safe sleep environment or car seat for a local infant.  Together, we can equip families with the knowledge and resources they need to keep their babies safe and save lives!

Delivering Change empowers families to achieve better maternal and infant health outcomes, through coordinated care, education, and connection to resources, helping healthy moms create healthy babies, leading to stronger families and a thriving community. 

Profile

 Delivering Change is a community drive initiative in Geary County that serves pregnant and postpartum women, fathers, infants and families. Founded in 2011, as a response to alarming infant mortality rates in Geary County, a group of local leaders and professionals developed the model of care that is now considered best practice today.  Through this model, Delivering Change has helped cut Geary County’s Infant Mortality Rate in half.  Delivering Change’s goal is to improve maternal and infant health outcomes by ensuring that every pregnant and postpartum woman, infants, and families have access to coordinated care, education and support needed. Through a four-pronged approach, focusing on Women’s Health, Infant’s Health, Breastfeeding, and Fatherhood, Delivering Change is breaking down barriers to healthcare, strengthening family support systems, providing coordinated and targeted education, and creating brighter futures for every baby born in our community.
